Registration E-Form

Registration is available only by filling out and returning the Registration E-Form (click here).
Please ensure all sections of the form are completed and returned to the CCMTA before April 5.

To register use the following steps:

1. Save the Registration E-Form PDF to your desktop.
2. Open and complete the Registration E-Form PDF.
3. Click the "submit" button on the lower right side of the Registration E-Form.

Please confirm registration prior to the deadline date. A full refund of registration fees will be issued for cancellation prior to April 5, a 50% refund of registration fees will be issued for cancellations up to April 13 and no refunds of registration fees will be issued for cancellations after April 27, 2012.

For Mac users:

• When the option menu appears, choose "Download link to disk" or "Download linked file"
  Choose the location on your computer where you would like to store the file.
• Your computer will start the download once you have selected a location
